Professional paper claims must be printed on the NEW CMS1500 02/12 forms starting 4/1/2014
Starting today, April 1, 2014, payers will receive and process only those claims that are submitted on the revised CMS-1500 claim form (version 02/12). As mandated by the Centers for Medicare & Medicaid Services (CMS), claims submitted on the old form (version 08/05) will no longer be accepted.
